Uploading evidence to plans (Personalised Planning)

This document will help you upload evidence to individual student plans (PPSD).

 

AN IMPORTANT NOTE ON UPLOADING EVIDENCE, PRIVACY AND STUDENT TRANSITIONS

The CSNSW PP tool now has the capacity for REPORT level access users to upload, attach & view evidence such as diagnostic assessments, specialist reports and other SENSITIVE, PERSONAL HEALTH information into a student’s individual personalised plan in NETiD. Whilst this is an exciting development for collation of evidence and the potential reduction of administration burden for users, it is pertinent that:

  • schools and dioceses are actively reminded of their obligations under relevant privacy legislation regarding the sharing and accessing of SENSITIVE, PERSONAL HEALTH information within the PP tool at the point of transitioning student personalised plans

  • schools and diocesan systems are actively reminded to ensure that have efficient processes in place to obtain and store written parental consent to request, obtain, share or access student personalised plans (and their associated attachments) from other educational settings

WHO CAN UPLOAD EVIDENCE TO STUDENT PLANS?

  • Any user that is registered for Personalised Planning within NETiD for their organisation PLUS has REPORT level access (not just STANDARD level access). Principals in schools have the access level to grant REPORT level access for staff in their schools

  • REPORT level access will allow users to see all three function buttons on the school Student Management page: Please refer to the CSNSW Personalised Planning help document on TRANSITIONING STUDENT PLANS for more detailed information regarding privacy legislation and requesting and/or accessing transitioned student plans.

  • Only those users with REPORT level access will be able to UPLOAD, ATTACH, OPEN A DOWNLOADED DOC and ARCHIVE any evidence document into/within the CSNSW PP tool

  • Those users with STANDARD level access will be able to see the thumb nail of the title of uploaded evidence but will not be able to OPEN, DOWNLOAD or ARCHIVE the document in the plan nor will they be able to UPLOAD any documents – all three buttons will appear faded and will not carry out any function when clicked on by a user with STANDARD level access

WHAT TYPE OF EVIDENCE DOCUMENTS CAN I UPLOAD INTO STUDENT PLANS?

  • ONLY PDF documents can be uploaded into student plans

  • A reminder only those with REPORT level access can upload documents

WHERE CAN I UPLOAD EVIDENCE TO STUDENT PLANS?

  • The only TAB currently that has the capacity for REPORT level access users to upload evidence is the “Personnel and Planning” TAB in the PP tool

  • Select a student from the Student Management page by clicking on the name

  • Select the Personalised Planning program for the student by clicking on the blue PPSD button:

  • Select the Personnel and Planning TAB by clicking on the TAB button at the top of the page:

  • Scroll down the page to the section “Evidence of Assessed Individual Needs of the Student”. There are five sections available here that display a heading, text of examples, a text box and a green button for adding supporting documents

HOW DO I UPLOAD EVIDENCE INTO A STUDENT PLAN?

For those with REPORT level access:

  1. Once you have followed the previous steps to reach the “Evidence of Assessed Individual Needs of the Student” section, select any of the following areas to upload evidence into:

    1. Diagnostic Information Examples: diagnostic assessments, letters of diagnosis from specialists, specialist reports, etc

    2. Imputed Disability Information Examples: summative school assessments, standardised assessments, profiles or assessment reports that identify functional needs etc

    3. Medication Information

    4. Technology equipment and environmental supports Examples: wheelchairs, AAC or communication devices, text to speech devices, ipads, mobility belts, hearing aids, braille, minor capital supports etc

    5. School Attendance Information Examples: attendance letters, flexible attendance timetables etc

  2. Click on the green “+ Supporting Documents” button

  3. Clicking on this button will automatically open the file server on your computer so that you can locate and select a file to upload:

  4. Once you have selected a file to upload it will appear as the following

  5. You can upload up to five (5) PDF documents into any one of the five areas outlined in point one. When you attempt to upload a sixth document, there will be text that appears to indicate that you will need to archive a document before adding another

  6. All five areas may have documents uploaded into them – therefore there is a maximum capacity of twenty-five documents that can be uploaded per plan.

PLEASE NOTE: It would be best practice and highly recommended that schools have saved and stored evidence documents in a secure location within a school’s own server AS WELL AS uploading evidence documents into each student’s plan within the PP tool.

HOW DO I REMOVE A DOCUMENT THAT I HAVE UPLOADED INTO A STUDENT PLAN?

For those with REPORT level access: To remove a document that has been uploaded into a student plan, click on the red ARCHIVE button

HOW DO I OPEN A DOCUMENT THAT I HAVE UPLOADED INTO A STUDENT PLAN?

For those with REPORT level access:

  1. To open a document that has been uploaded into a student plan, click on the blue DOWNLOAD button

  2. This will automatically download the selected document onto your computer so that it can be opened

HOW ARE THE UPLOADED EVIDENCE DOCUMENTS DISPLAYED IN THE INDIVIDUAL STUDENT PPSD REPORT?

When any user (STANDARD or REPORT level access) generates an individual student PPSD report, the following information displays in the report:
