Select a category from either Physical, Cognitive, Sensory or Social/ Emotional.

Click Confirm and Go to Step 2
Select the Adjustment Level
Scroll down to the bottom of the page
You will be shown 2-3 boxes to help you confirm the level of adjustment.
The first box tells us that you agree with what you’ve clicked above.
The second box tells us that you disagree with what is above, and you believe the level of adjustment is less than what is above.
The third box tells us that you disagree with what is above, but you believe that the level of adjustment is more than what is above.
Click on the “Yes”, “Close”, or “No” button within one of the boxes above
Click CONFIRMED
