  1. Go to CSNSW Service Portal https://csnsw.atlassian.net/servicedesk/customer/portals

  2. Select Data Request in CSNSW School & Diocese Service Desk under Suggested forms

  3. Fill up the form with the following details then click Send

  • Summary – Enter a short description of the request.

  • Description - Add specific information such as;

    • What information is needed?

    • When is it need?

    • Which school, year, or diocese the data will be from?

  • Business Justification – Provide the reason or purpose of the data request

A captcha window will come up and select the correct images as requested. Click Verify after all correct images have been selected.

After the images are verified, the next page will show your reference/ticket number for tracking and updates.
